Help Brad Live In His House Indepenently

My name is Brad and I have a rare severe deteriorating neurological condition. I suffer from multiple seizures per day and am regularly hospitalised due the the physical damage done to my body from falls. I have suffered from this for years and my medical condition is worsening. I want to be able to live in my own home and to keep my independence I require access to community organisations that support disabled people.
I am appealing to my fellow human beings genuine compassion seeking monetary contributions to raise funds quickly to modify my home to suit my medical requirements and to pay for care workers to provide me the support needed to be able to live in my own home, grocery shop, provide physical care, cook meals, personal hygiene and not be put into a nursing home as I am only 56. I want to keep my independence that 99% of people take for granted in their lives.
